Registration of New Patients
All new patients are invited to make a 20 minute appointment with the doctor or practice nurse. If you are taking regular medication you will need to see the doctor before ordering your first repeat prescription.
You may choose to see any of the GPs in the practice for routine care.
Practice Questionnaire
You will be asked to complete a questionnaire relating to your past medical history. This will help ensure that all relevant aspects of patients care are covered at your initial consultation. You will also be asked to bring a specimen of urine for routine testing.
We recommend that children from 0 - 5 years old are registered with our child development and health screening programme. It is also important that you give us details of your child's immunisations with dates. This ensures your child completes a full immunisation schedule.
It would help if you could complete the 'women only' section as accurately as possible, you will then be entered on the recall lists.
